เงื่อนไขการให้บริการจองวัคซีนทางเลือก Moderna
(เป็นเงื่อนไขในหน้าแรกที่ท่านได้กดยอมรับ ก่อนทำการจองวัคซีน)
ตามที่ "รัฐบาลได้จัดสรรวัคซีน โควิด-19 ให้กับประชาชนโดยไม่มี ค่าใช้จ่าย" และเปิดโอกาสให้โรงพยาบาลเอกชนสามารถสั่งซื้อวัคซีนทางเลือก Moderna ป้องกันโควิด-19 ผ่านองค์การเภสัชกรรม เพื่อเป็นทางเลือกหนึ่งที่ประชาชนสามารถเลือกรับบริการได้ แต่ต้องชำระค่าใช้จ่ายเอง โรงพยาบาล บี.แคร์ เมดิคอลเซ็นเตอร์ เปิดจองวัคซีนทางเลือก Moderna โดยชำระเงินเต็มจำนวนล่วงหน้า คาดว่าจะได้รับวัคซีนประมาณไตรมาสที่ 4 (ต.ค - ธ.ค 2564) ถึง มกราคม 2565 โดยสมาคมโรงพยาบาลเอกชน ได้มีมติเมื่อวันที่ 2 ก.ค. 2564 กำหนดราคาวัคซีน เข็มละ 1,650 บาท (2 เข็ม ราคา 3,300 บาท) โดยเป็นราคาสุทธิที่รวมค่าวัคซีน ค่าบริการโรงพยาบาล และ ค่าประกันวัคซีน จึงใคร่ขอให้ท่านยืนยันความต้องการ เพื่อทางโรงพยาบาลจะดำเนินการสั่งจองวัคซีนต่อไป
วัคซีน Moderna เป็นวัคซีนโควิด-19 ประเภท mRNA ฉีดในผู้มีอายุ 18 ปีขึ้นไป ฉีด 2 เข็ม ห่างกัน 28 วัน นำเข้าจากประเทศสหรัฐอเมริกา
*เงื่อนไขการจองวัคซีนทางเลือก Moderna
• ราคาวัคซีนเข็มละ 1,650 บาท / 2 เข็ม ราคา 3,300 บาท
• ทางโรงพยาบาลคาดว่าจะได้รับวัคซีนจากองค์การเภสัชกรรม ประมาณไตรมาสที่ 4 (ต.ค - ธ.ค 2564) ถึง มกราคม 2565 และจะให้บริการฉีดโดยเร็วที่สุด
• โรงพยาบาลจะจัดสรรวัคซีนให้ตามลำดับคิว สำหรับผู้ที่ชำระเงินเต็มจำนวนล่วงหน้า เนื่องจากวัคซีนมีจำนวนจำกัด
*การจัดสรรวัคซีน
เนื่องจากโรงพยาบาล ไม่ได้เป็นผู้จัดหาวัคซีนโดยตรง จึงอาจเกิดเหตุการณ์จำนวนวัคซีนที่ได้รับมาไม่เพียงพอต่อจำนวนสั่งจอง โรงพยาบาลขอสงวนสิทธิ์ในการจัดสรรวัคซีน ให้ผู้ที่ชำระเงินเต็มจำนวนตามลำดับ
*เงื่อนไขการคืนเงิน
1. กรณีที่โรงพยาบาล ไม่ได้รับการจัดสรรวัคซีนตามที่ได้ทำการจองไว้ ท่านสามารถเลือกการดำเนินการได้ ดังต่อไปนี้
1.1 ขอรับเงินคืน ตามจำนวนที่ได้ชำระเข้ามา โรงพยาบาลจะทำการคืนเงิน ให้กับผู้ที่ไม่ได้รับการจัดสรรวัคซีน ภายใน 15 วัน หลังจากที่ได้รับทราบผลการจัดสรรและได้รับรายละเอียดการโอนเงินจากผู้จอง
1.2 แจ้งความประสงค์รอรับวัคซีนใน lot ต่อไป
2. หากทางโรงพยาบาลฯ ได้รับวัคซีนมาแล้ว ท่านไม่สามารถมาฉีดได้ไม่ว่าจะด้วยกรณีใดก็ตาม ขอสงวนสิทธิไม่คืนเงิน แต่ท่านสามารถโอนสิทธิให้บุคคลอื่นได้ โดยแจ้งล่วงหน้าก่อนถึงกำหนดรับวัคซีนครั้งนั้น
*การนัดหมายเข้ารับการฉีดวัคซีน
หลังจากได้รายชื่อผู้ที่ได้รับการจัดสรรแล้ว ทางโรงพยาบาลจะแจ้งรายละเอียดการนัดหมายการฉีดวัคซีนให้ทราบทาง SMS และช่องทางออนไลน์
Terms of service for reservation alternative vaccines Moderna

Moderna vaccine is mRNA-type COVID-19 vaccine which is administered in people aged 18 years old and above by injecting 2 doses 28 days apart. The Moderna vaccine is imported from the United States.
*Conditions for reservation alternative vaccines Moderna
1. The price of vaccine at 1,650 baht per dose (total price 3,300 baht for 2 doses).
2. The hospital expects to receive the vaccine from the Government Pharmaceutical Organization around the 4th quarter of 2021 to January 2022. and will proceed for injection as soon as possible.
3. The hospital will allocate the vaccine according to the queue for those who fully paid in advance due to the limited number of vaccines.
*Vaccination allocation
Due to the hospital is not a direct supplier of vaccines. Therefore, it may occur that the number of received vaccines is insufficient to meet the number of pre-orders. The hospital reserves the right to allocate vaccines to those who fully paid in order.
*Refund conditions
1. In the case of the vaccine was not allocated to hospital as reserved, you can choose an action from the following methods.
1.1 Request to refund based on the amount paid. The hospital will refund to persons who have not been allocated the vaccine within 15 days after receiving the allocation results and receiving the remittance details from the booker.
1.2 Request to wait for vaccination in the next lot
2. If the hospital has been allocated the vaccine, but you can't come to receive the vaccination under any circumstances. The hospital reserves the right not to refund but you can transfer this rights to other people by notifying in advance before the due date for that vaccination.
*Appointment for vaccination
After receiving the list of allocated persons, the hospital will inform the details of the vaccination appointment via SMS and online channels.
